You are requested to complete this form because you have expressed interest and need for our Bookkeeping Services. Please provide the information and documents requested in this form, so we may begin the onboarding process for your business. All information that you provide is strictly confidential and under AICPA Code of Professional Conduct (AICPA Code) Rule 1.700.001, as a member in public practice we shall not disclose any confidential client information without the client's specific consent.

IMPORTANT: Estimated average time needed to complete this form is 20 minutes. Please complete this form in one session. Please complete this form in one session. Prior to completing the form, you may need digital copies of the following documents (not all may apply to your business):

• FEIN Registration Letter (Form SS-4)
• State Employer Registration Confirmation with business registration number for each applicable state
• If elected S-Corp tax treatment - IRS confirmation letter
• Scan/picture of VOIDED CHECK - your payroll bank account and all direct deposit employees' bank account
• Filled out Form W-4 for each employee to be added to payroll
• Filled out Form I-9 for each employee to be added to payroll
• Direct Deposit Authorization From for each payroll employee with direct deposit election
