Please provide the information and documents requested in this form, so we may begin the onboarding process for your business. All information that you provide is strictly confidential and under AICPA Code of Professional Conduct (AICPA Code) Rule 1.700.001, as a member in public practice we shall not disclose any confidential client information without the client's specific consent.

• FEIN Registration Letter (Form SS-4)
• If elected S-Corp tax treatment - IRS confirmation letter
• Scan/picture of voided check - all business checking and payroll bank accounts
